Initial deposit to schedule

  • We ask for a $100 deposit to schedule.  This deposit will apply to the total cost of the the 60 minute appointment.  The total cost is $300. 

  • We appreciate this can be a barrier but we use this approach to ensure patients are ready to invest in their health and avoid no-shows for appointments.

How to make an appointment

  • Make an appointment online - Use our booking site to make your first appointment.

    • Click on "Consultation - Fulcrum Health & Wellness"​ 

    • Choose your preferred clinician.

    • Choose your preferred appointment type.  Please note consultations are designated as "Nonmember." All other appointments are for DPC members.

  • Call our office - Prefer the phone?  Give us a call and we'll help you find an appointment.  Phone # 207-386-4895.

Gathering data

  • You will receive intake forms in your email.  Please fill out the forms as best you can.  We will discuss your answers in detail so please don't worry if you feel you have more to explain. 

  • Please feel free to bring previous clinical information to your appointment for review.
